At the eave, the covering and edge metal should direct rain into the gutter. Displaced metal can send water behind it and onto the fascia, even when the gutter itself has no hole.
Along a supported run
A dip can leave water sitting between brackets instead of moving toward the downspout. The brackets and the wood behind them need to hold the gutter at the right line.
Through the outlet
Debris can collect where the gutter narrows into the downspout or at an elbow below. Clearing only the visible top leaves that restriction in place. A guard above the gutter does not establish that the entire outlet is open.
